After your order has been submitted, South Creek HR will contact you to obtain the names and email addresses for each person you have ordered an The Five Behaviors Profile for. Following, those individuals will receive an email with a link to complete an online assessment. The online assessment typically takes 15-20 minutes and the results produce their personalized Five Behaviors Profile.
Based on the work of Patrick Lencioni’s international best-seller The Five Dysfunctions of a Team, The Five Behaviors of a Cohesive Team® Powered by Everything DiSC® helps teams apply The Five Behaviors™ model of Trust, Conflict, Commitment, Accountability, and Results to drive team effectiveness and productivity. The program uses Everything DiSC, a tool that helps build effective team communication and relationships, to help participants understand their individual DiSC® style and discover how each style brings value to the team.
What does this program do?

Accountability, and Results. Individual team members will learn about their own personality style and the styles of their team members—based
on the Everything DiSC® model—and how their style contributes to the team’s overall success.

Program Component #1: The Five Behaviors Powered by Everything DiSC Profile
The program helps teams understand how they score on the key components of The Five Behaviors model: Trust, Conflict, Commitment, Accountability, and Results. Individual team members will learn about their own personality style and the styles of their team members—based on the Everything DiSC® model—and how their style contributes to the team’s overall success.
Facilitation: 7 modules with over 40 minutes of video and annotated facilitator reports
Assessment: 3 sets of questions—full, adaptive version of Everything DiSC, team survey, and team culture
Individual Profiles, Team Progress Reports, and one-on-one Comparison Reports

The Five Behaviors™ Facilitator Accreditation
Gain the confidence and expertise to transform talented individuals into extraordinary teams. The Five Behaviors of a Cohesive Team® Facilitator Accreditation is a blended-learning course that will help clients hone their skills as a facilitators of this unique program. Your clients will earn the credential of a Five Behaviors Accredited Facilitator, gaining expertise in using the model and accompanying course material.
Although the course uses The Five Behaviors of a Cohesive Team®, powered by Everything DiSC®, the credential also applies to use of The Five Behaviors, powered by All Types™. If you are interested in learning about the All Types version of the profile, contact South Creek HR.
